Here are three simple things you can do before we talk. This will help us make the most of our time together:
Step 1 - Check your Medicare Card
What to do: Make sure you have your red, white, and blue Medicare card. This card shows you have Medicare Part A and Part B.
Need to sign up for Medicare? If you don’t have your Medicare card yet, you will need to sign up for Part A and Part B, you can do it on the Social Security website or by calling 800-772-1213. Here is the link:
Step 2 - Help Us Prepare For Your Meeting
We need some details before your upcoming meeting. Please click the button below to get started. You’ll be asked to provide the following:
1. Doctors, Pharmacies, and Hospitals
Please list your doctors, preferred pharmacies, and any important hospitals. This will help us match plans to your current providers and keep your care in-network.
2. Current Medications
Share your list of prescription medications. This will allow us to estimate your total drug costs for each plan and identify options that best suit your needs.
3. Scope of Appointment Form
Medicare rules require this form before we can discuss plan options with you. It permits us to discuss Medicare Advantage, Part D drug plans, and Medicare Supplement plans.
Initial next to any type of plan you may want to hear about. It’s best to initial more plan types, not fewer, to give us the flexibility to cover all options that might be a good fit.
This does not lock you into anything
Most appointments require submitting the form at least 48 hours before your meeting.
