Here are three simple things you can do before we talk. This will help us make the most of our time together:
Step 1 – Check your Medicare Card
What to do: Make sure you have your red, white, and blue Medicare card. This card shows you have Medicare Part A and Part B.
Need to sign up for Medicare? If you don’t have your Medicare card yet, you will need to sign up for Part A and Part B, you can do it on the Social Security website or by calling 800-772-1213. Here is the link:

Step 2 – Complete a Quick Form (Scope of Appointment)
What it is: There is a form called a “Scope of Appointment.” It’s a rule from the government that helps us know what you want to talk about. We can only talk about the things you choose on this form. Please check all the boxes that you want to discuss.
What to do: We will send this form to you as a link in a separate email. Please fill it out and send it back to us before we talk. If you need help with the form, just ask us!
